Student Behavior Form

    I've been trying to create a form using Excel ('cause I know how to use that). I want a form that basically will be a running record of student behavior - to keep track of the steps I've taken.

    I want a a space at the top for Student Name and Period. Then, I want a short-hand spot for me to indicate dates of: "VW" (Verbal Warning), "TM" (Team Meeting), "Spoke with student one-on-one" (what do I call that), "PC" (phone call). I also want some room to write notes... like what the parent told me regarding student's behavior or even if I called and the number has been disconnected, etc.

    So I need a spot for "notes".

    I don't want to re-invent something... I'm willing to share what I've made so far, but it's just not tweaked up enough for what I want.

    Does anyone use such a form?

    Spoke with student 1:1, I call conference (C).

    I don't have a form like this but I do have the students fill out a form each time I need to speak with them about behaviors. Then I circle the consequence which is similar to what you are talking about (warning, conference, phone call/email, etc).

    I file these for later reference if need be.

    Mopar - yes, I thought about that, too. I wanted to have one 'running record' type of form, though, so everything is in one spot.

    Am hoping someone has created something!
