    I am doing yet another application and they ask for my cooperating teachers name,school, addy, and phone. Then they want 3 references. Should I not put her down since they already asked all her information earlier in the application?

    You do need her down in the reference section, IF you want her to be one of the 3. Now! If you don't want her to be one of the 3, then get someone else. You can always check with the people who gave you the application and find out if she can also be used as one of the 3.

    Yes, you can put your cooperating teacher down in both spaces.

    Yes...she goes in both spots. And...if you can get her/him to be in one of those spots, get your student teaching principal as well.

    Here's what I have:

    References
    1) Both cooperating teachers (a job-sharing team)
    2) Student teaching principal
    3) Clinical experience (pre-student teaching) mentor

    Letters of reference
    1) Student teaching evaluations/Cooperating teachers' letter
    2) Student teaching evaluation/University Supervisor's letter
    3) A teacher's letter (she and I co-taught summer school)

    I have something similar
    References:
    1.Teacher whose class I am in this year as an instructional assistant
    2.The Youth and Fam Director from the YMCA (I work at a summer camp there)
    3. Student teaching cooperating teacher
    4. Principal at the school I'm at right now as an IA

    Letters
    1. Same as previous mention
    2. college supervisor for s.teaching
    3.cooperating teacher

    I normally put my cooperating teacher down for reference and then at the student teaching part (obviously). I guess today I just felt weird typing the same stuff twice and started doubting if I should have it all down again. But yea I am def. using her as a reference thanks :)
