Returning Student Parent Survey?

    Last year as a new teacher I did parent surveys for all my kids to get basic contact preferences from parents but also information about their child (strengths, weaknesses, goals etc.). I loop with my kids for the 3 years they are in middle school so I will know the 7th and 8th graders this year already.

    The surveys were a big hit with parents last year and really opened communication so I would like to do them again, but what kind of questions would you ask for students I already know?

    Have there been any changes in your family structure in the last year? (New babies, divorce, remarriage?)

    Have there been any medical issues I should be aware of?

    Good ones! So in addition to those two, here are three that I had on last year that I think would still be applicable (or maybe I should change the wording to reflect it being for the current school year...)

    What are your child’s strengths academically?
    What would you like to see your child improve on academically?
    Is there anything else I should know about your child to make this year in science more successful?

    What are your child's interests outside school?

    Are there any food allergies your child may have? If so, how would you like me to handle classroom parties, etc?

    How does your child feel about school in general?

    How does your child like to be addressed (e.g. "Michael prefers to be called Mikey")?

    Does your child attend any activities each week after school? (Ex. Girl Scouts, karate, Religious Functions) Which night are these functions held? (Ex. Every Wednesday Night)

    What would your child describe as the best part of his/her experience in science class last year? What was his/her least favorite experience?
