    Would it be wrong to a send a gift card to my cooperating teachers as a thank you for completing district reference forms?

    There is definitely nothing wrong with this. It is not required, but would be a nice gesture.

    I would send a gift to the cooperating teacher, but I would specify that it's for the help he/she gave during your student teaching...to me, it sounds a little fishy to monetarily thank people for filling out reference forms for you, no matter how innocent the intent may be.
