Reading Notebook Reflection

SOMEWHERE, on someones site I saw a reflection sheet to fill out after reading workshop, well now I can't find it. Does anyone know what I am talking about or where I saw it?

This sounds like something I would like for my own files since I would eventually like to teach 4th grade.

 

i don't remember all of it but i know it had questions like did you enjoy what you read, did you spend your time wisely, it was really short, like half page...i wish i could remember

 

do any of you have a reading response journal sheet, with possible questions to respond to??

I have a reading response log and list of questions to go with it. My students are required to read for 15 minutes and answer one question a night about what they read. They must choose a different question each night i.e. Character, Plot, Setting etc. If interested PM me or email me at MooreSVMK@aol.com. (This actually was given to me by a friend!)
