prewriting/research student form

    I was sifting through the many web pages out there about teaching the stages of writing several months ago when I found a teacher's page with a word document I really should have printed and/or saved immediately. Instead I thought "Oh that's cool," and then carried on my search (for something else) without so much as bookmarking the dang page. Now I'm kicking myself because I could really use that page. The teacher had created a form for students to use while researching on the internet for their prewriting stage. It had a space for the topic, questions the student had about the topic, lines to record the "who, what, where, when, why, and how" info, then maybe some space for an illustration. Anyone use something like this or know what I'm talking about?
