In need of a graphic organizer for state report?

Do any of you know of where I can find a good graphic organizer and/or planning sheet for 4th graders to use to write a report on a state?

 

Is there any particular focus to the report, or do you want a bit of everything?

No particular focus and each student will have a different state that they are researching, I was thinking of something to help them direct their research (capital, population, interesting info, etc)
