First day information sheets

    I'm a new 7th grade math teacher, and I was wondering if anyone has the students fill out information sheets on the first day, and what kinds of questions you ask. There are the basics, such as parents names, phone numbers, etc ( I could get this from the office but I hear it takes them a while to get the info to the teachers). Then there are things to get to know the students better, such as what do they like about math, what do they not like, etc. What do other teachers do?

    On the first day I have the students complete an info sheet about themselves so I can get to know them. I also send home an info sheet for their parents to fill out about their child plus contact info. If you want I can e-mail you the sheets I use.

    I ask students to fill out similar information sheets. Here are some of the questions I make sure to ask:

    • Who do you live with?
    • Does your parent/guardian speak English?
    • What are your hobbies/interests outside of school?
    • Do you babysit younger brothers or sisters?
    • What class are you most excited about this year?
    • What class are you least excited about this year?
    • Where in the classroom do you like to sit? Front, middle, back, no preference
    • How do you prefer to work? Alone, with a partner, in a group, doesn't matter
    • Do you like to listen to music while you work?
    • What can I do to help you be successful in our class?
    • What else should I know about you?

    This will be my first year teaching, but here's some questions I plan on handing out to my math students during the first couple days of school:

    My best kept secret about math is...
    If math could be a color, it would be...because...
    When it comes to math, I find it difficult to...
    When it comes to math, I am very good at...
    Does math or math class scare you in any way? Why or why not?
    One way I used math over the summer was...

    I save these each year and mail them to the seniors I had when they graduate and they really appreicate reading these again.

    Might not be applicable to everyone, but i also ask what language is spoken at home.

    That's a really good question.

    I ask whether a parent speaks English because that's how I determine the means by which I will contact home. If I can't speak with the parent on the phone, I'll have to get a translator (which is easy if the home language is Spanish, more difficult if it's something else).

    I also am new but I'm asking them about their personal life (middle schoolers love to talk about themselves) but also, "One of the best assignments I remember from last year’s math class was…", and Things about How I can help them learn math such as "It confuses me when a teacher does" and "It helps me when a teacher does..."

    One question I read about last week that sounds like a good idea is to ask them to pretend they're having a birthday party and can invite only 3 friends from this class--who would they be? Then you can use that info to make seating charts and identify the popular ones (leaders). I can't remember if I got that from Wong or Teaching with Love and Logic.
