Behavior Plan

For step three on my consequences, I am wanting to create some type of behavior plan for the student to fill out talking about what behavior(s) are disruptive, why they are disruptive, create a plan to solve the problem and how this will help the classroom as a whole. This will be signed by the student, myself and the parent(s). Does anyone have an example of this or use something similar? I can't decide how to word it, format it, etc... Thanks!
