1:1 individual support rubric?

    Does anyone have a good rubric their district uses for team discussion for determining need for 1:1 adult support?

    I'm specifically looking for one that covers all areas of the day - functional, self-help, group skills, specials, lunch, recess, arrival, etc...but I'd love to anything other people use.

    We don't use a rubric. Instead we have data demonstrating the need. For instance, one of my students needed a 1:1 adult for PE because he was hitting other students daily. Another students needed 1:1 support for lunch because he was licking the floor. I don't even know how you would show these two different behaviors on the same rubric.

    We just kept a log for about 2 weeks of the behaviors to demonstrate that they were happening more than daily. That was enough for the administration.

    I was looking for a similar rubric last year, and nobody could help.
