Does anybody use Think Sheets or Choices Cards as part of their behavior management? I was thinking that students displaying poor behavior would have to fill out a sheet saying what they did, why it was a poor choice, what a better choice would be, etc. Then they would sign it, I'd sign it, and a parent would have to sign and return it. I would follow up with (hopefully) positive notes about behavior improving, or if the poor behavior continues, a phone call home to discuss it. Does anyone do this and, if so, has it been effective? Also, if anybody has any documents that they use and wouldn't mind sending them to me or linking me to one on the 'net, I'd appreciate it!

Our school uses think sheets when the students are sent to the "safe seat". We follow BIST, so it is manditory. Parents are to sign the sheet, but I always make a copy first, since 10 yr. olds seem to lose everything!
